I hereby make application to the Sanctioning Authority of Boxing Nova Scotia for a sanction under the rules of Boxing Canada

We hereby agree to:
1. Be registered with Boxing Nova Scotia
2. Abide by the rules of the Boxing Canada
3. Use only officials who are registered members in good standing of Boxing Nova Scotia
4. Pay Boxing Nova Scotia the sum of $300.00 to cover the cost of the sanction
5. Be responsible for all legal and financial responsibilities for the event covered under thissanction
6. Furnish all documents required by Boxing Canada within seven (7) days of the event to the Chief Official of the Province

NOTE: This application must be completed in full and received at least THREE WEEKS PRIOR TO THE EVENT date with the appropriate fee.
